The Board has made every effort to include the information you need to renew your license on this website. Visit our Help Center and Resources page for frequently asked questions, links, forms, applications and other helpful information. If you have additional questions about your renewal process, please contact a renewal specialist.
Current licenses expire at midnight, Eastern Time, on November 30, 2013.
To ensure you receive your renewal notification from the department, your current mailing address must be on file. Failure to renew an active or inactive license by the expiration date will result in the license being placed in delinquent status. Failure by a delinquent licensee to renew before the expiration of the current licensure cycle renders the license null and void without any further action by the board or the department.
A licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements.

The department will renew your license upon receipt of:
- Completed renewal application
- Required fees
By submitting the completed renewal application along with the appropriate renewal fees to the department, a licensee acknowledges compliance with all requirements for renewal, including continuing education.
- Login to DOH account at www.FLHealthSource.gov
- Follow on screen instructions for reporting CE hours to CEBroker.
- Once all hours are logged, the Department will allow your license to be renewed.
- Follow the on screen instructions for submitting payment and completing your renewal.
- New licenses will be mailed to your current address on file within 10 business days.
Certified check or money order should be made payable to the Florida Department of Health.
If renewing BEFORE your license expires, your fees will be:

*After renewal closes, a $35 “Delinquent” fee will be added to all transactions. A “Change of Status” fee will also be added for licensees wishing to change from Active-Inactive in the amount of $40.

First Biennium Renewal: Physical Therapists and Physical Therapist Assistants renewing a license for the first time must complete 1 hour on HIV/AIDS and 2 hours of prevention of medical error
HIV/AIDS courses required for first renewals are accepted if approved by any Board within the Division of Medical Quality Assurance
Board Rule Approved Providers
- Florida Physical Therapy Association (FPTA)
- American Physical Therapy Association (APTA) or any of its components. This includes any state
- Physical Therapy Association. This does not include other state board offices
- Courses sponsored by a CAPTE accredited PT/PTA program. One credit hour is the equivalent of one contact hour
Other Methods of Obtaining Continuing Education per Biennium
- Attendance at a Florida Board of Physical Therapy meeting (this does not apply if the licensee is on the agenda or appears for another purpose)
- Board members serving on a Probable Cause Panel
- Passing the Florida Laws and Rules exam within the biennium. This does not apply if taking the course due to a disciplinary order of the Board, initial licensure, reactivation or reinstatement
- Course Instructors can receive one contact hour for each hour presented
- No more than 12 hours can be earned by home study/online courses
